Headey made her feature film debut in 1992's Waterland, playing the younger version of Jeremy Irons' wife. She was also seen that year in the BBC television movie The Summer House.

In 1993, she appeared in the period drama Century and the award-winning Merchant-Ivory film The Remains of the Day.

Her first leading role came in the live-action version of Rudyard Kipling's The Jungle Book. She has starred in the independent films Mrs. Dalloway, Face, Onegin, Gossip, Possession, Ripley's Game and The Actors.

In 2006 she starred in the box office hit film 300, as Queen Gorgo. Zack Snyder directed Headey opposite Gerard Butler in this film about the real-life battle of Thermopylae, in which King Leonidas (Butler) and 300 Spartan warriors fought the massive Persian army, leading to Greece uniting and giving rise to democracy.

In 2007 she played a teacher, Miss Dickinson, in St. Trinian's, the contemporary update of the popular 1950s film franchise about an all-girls' school. The film also stars Rupert Everett and Colin Firth.

In 2008 she appeared in the psychological horror film The Brøken, in which she stars as a woman whose life is thrown into turmoil when she sees her exact double on the streets of London, and The Red Baron.

She co-produced and appears in the short film The Devil's Wedding by director Dan Cadan, alongside the star of This is England, Steve Graham. She also stars alongside Jamie Foreman and Bill Sykes in Roman Polanski's Oliver Twist.

Her previous credits include the horror thriller The Cave, the fantasy-adventure The Brothers Grimm and the British romantic comedy Imagine Me & You.
